
Post 61: Is Your Child's Standing Frame Really Helping? What Every CP Parent Should Know
If your child uses a standing frame, you've probably watched the clock during those sessions and felt a certain relief once you hit the 45-minute mark. It's on the schedule. It's checked off. Weight is going through the legs, the hips are loading, the bones are getting what they need. That's the theory, and it's a reasonable one to hold onto.
But here's a question worth sitting with: what if standing still in a frame for 45 minutes isn't just falling short of the benefit you're hoping for — what if, in a body that isn't organized yet, it's actually working against you?
I know that's not what anyone wants to hear after all the effort that goes into fitting a frame into the day. But it's worth understanding, because it changes how you think about standing altogether — not by adding more of it, but by rethinking what kind of standing actually helps.
The Legs Aren't the Only Thing Doing the Work
In a standing frame, it's the legs that are technically weight-bearing — feet on the ground, load traveling down through the hips and knees. So it makes sense to assume that's where the "work" is happening, and that's where progress should show up. This is the same tension I explored in the Standing Frame Problem — the assumption that legs bearing weight automatically means the body is benefiting.
But weight-bearing in the legs doesn't happen in isolation. For that load to actually organize the body — to build the bone density, joint stability, and proprioceptive feedback we're hoping for — the trunk above it has to be doing its job too. The connection between the ribs and the pelvis, the flexibility through the spine, the way the torso holds itself upright: all of that has to be organized enough to receive and pass along what's happening underneath.
This is the same principle I talked through recently with a family in TheraParent Coaching, when they noticed their daughter's trunk getting strong while her arms weren't following. The lesson applies just as directly here: if the structure above isn't ready, the part below it can't simply be held in place and expected to benefit. Something has to give — and in a static frame, what usually gives is the child's own structure, straining to hold a position it isn't ready to hold on its own.
What This Actually Means
I don't recommend standing frames, and this is exactly why. A frame holds a child still in one fixed position for an extended stretch of time. If the trunk hasn't organized enough to support that position yet, the frame doesn't wait for the body to catch up — it holds the shape regardless, and the child's structure ends up compensating to stay there. That's not building strength. That's forcing a static shape onto a body that isn't ready for it.
Here's where this becomes easier to see. Many children placed in a standing frame need their legs straightened to fit the frame's shape, even when their body's natural tendency is to bend at the knee. Many need an AFO to keep the foot flat against the footplate, even when the foot wants to hold a different position. Many need tight straps around the pelvis and hips just to keep them from folding forward, because without that support, the tendency is to collapse into flexion.
Each of those adjustments is being made for a reason — to get the child into the position we think standing requires. But look at what that actually tells us. If we have to straighten the leg, flatten the foot, and strap the hips in place to hold a shape, the body is telling us, quite clearly, that this isn't where it currently organizes itself. From the outside, the result can look tidy — legs straight, feet flat, pelvis upright, everything lined up the way standing "should" look in a photo. But that alignment isn't coming from the body's own structure. It's being held there externally, against what the tissue is doing on its own.
And when a position has to be applied rather than supported, the load doesn't travel the way we intend it to. Weight-bearing only creates its benefits — bone density, joint stability, proprioceptive feedback — when the load moves cleanly through a structure that's organized enough to receive it. If the knee is being held straight instead of settling there naturally, if the foot is locked flat instead of finding its own contact with the ground, if the pelvis is strapped upright instead of resting there, the tissue underneath each of those points is working to resist the position rather than support it. The load gets absorbed by that internal struggle before it ever reaches the bone, the joint, or the proprioceptive system we were hoping to reach. It looks like weight-bearing. Structurally, it isn't quite arriving.
What I teach instead, both here and inside TheraParent Coaching, is smart standing — dynamic, supported standing that meets the child's structure where it actually is right now, rather than adjusting the child to meet a fixed shape. Instead of straightening, flattening, and strapping a child into alignment, smart standing uses movable, adjustable support — a ball, a wall, a chair, a hand — so the child can be upright and weight-bearing in whatever position their body currently organizes around, while staying in gentle motion instead of locked in place.
The difference isn't about how long a child stands. It's about whether standing invites the body to organize itself, or requires us to hold it in a shape it hasn't built the support to hold on its own.

What This Can Look Like at Home
You don't need special equipment to start shifting this. A few ideas:
Instead of static standing, use a large ball for support so your child can lean, shift, and be gently rocked — weight-bearing happens through motion instead of a held position
Notice where your child seems to be bracing hardest while upright — often it's not the legs at all, but the trunk trying to compensate for support it doesn't have yet
Pair any standing time with small, continuous movement — side to side, forward and back — so the position stays active and responsive rather than fixed and static
Alongside this, continuing the fascia work on the trunk, spine, and the connection through the ribs and pelvis is what actually builds the foundation smart standing depends on.
The Reframe
It's tempting to think of standing as something to accumulate — more minutes, more frequency, more consistency, and eventually the strength will show up. But standing is only as beneficial as the body's readiness to be in that position. A static frame doesn't ask whether the body is ready. Dynamic, supported standing does.
The goal isn't more time standing still. It's standing that moves with your child's body, so what looks like progress on the outside is actually organization happening on the inside.
